CLIENT DISCLOSURE – Emotional Freedom Technique (EFT)
Client recognizes that the Emotional Freedom Technique (EFT) practitioner is
not a licensed physician, psychiatrist or psychologist. The American Medical
Association accepts EFT; however, it is not licensed by the State of California,
nor are energy therapies. All services provided are done with the utmost
concern for the client’s well-being and in good faith. Results of the sessions
are ultimately the client’s responsibility. This assumes all information provided
by the client is true and complete. If you ever have any concerns about the
nature of your treatment, please feel free to discuss it. In order to work with
medical conditions, I must receive a release from your physician. It is
recommended that you inform your medical doctor that you are receiving EFT.
In order to use these services, California State law requires that you
acknowledge receipt of the information provided in this form and that you sign
it. You will receive a copy. Copy will be kept on file for at least three years.
EFT is an alternative or complementary to healing arts that are licensed by the
State of California. Under Sections 2053.5 and 2053.6 of California’s Business
and Professions Code Devina can offer you these services.
​Devina attained her certification in foundational EFT training
in March 2018 with Ms. Lynn Whitmire who studied with the founder of EFT,
Gary Craig and also conducts basic and advanced EFT workshops.

EFT Seminars

Group Conference Calls Disclosure

By participating in our online EFT Seminars or group conference calls, you understand you will be learning about and using Emotional Freedom Techniques (“EFT”). The premise of EFT is that the flow and balance of the body’s electromagnetic and more subtle energies are important for physical, emotional, mental, and spiritual well-being. EFT uses the Chinese acupuncture meridian system to balance the subtle energies with a gentle tapping procedure which stimulates particular meridian points on the face and body.

Since EFT is a relatively new healing approach and the extent of its effectiveness, as well as its risks and benefits, are not fully known, you agree to assume and accept full responsibility for any and all risks associated with using EFT with yourself or others. In addition, you assume and agree to accept full and complete responsibility for applying what you may learn from participating in these events.

You understand your participation in the EFT events is not a substitute for medical or psychological treatment. You agree to take full responsibility for your self-care in the emotional, mental, physical, and spiritual dimensions of your life and to seek professional advice as appropriate before making any health decisions.

Since the seminars and conference calls involve group processes in which you may voluntarily reveal personal information, you understand that you waive your rights of privacy and confidentiality. If you inadvertently experience any distressing reactions by participating in the seminar/conference call you agree to let the Presenter know.
